Our Nurses and Case Managers are your Health and Safety Advocates.Your Health and Safety at Home are our Number One Priority.
Do you know why you take all the medications you do? Do you know the possible risks and side effects that may be affecting you? Do you know what questions are important to ask of your health care providers? These are just some of the issues you will be able to discuss with your Skilled Nurse or Case Manager.
Alternately, you may schedule a Consultation with one of our Health and Safety Advocates to meet with you one on one, to answer questions you may have that seem overwhelming, or even to accompany you to your medical appointments. It is important that you get the information you need to feel good about your health care decisions.
