DISEASE BASED HOME SUPPORT
Depending on your personal condition, your Health Care Providers may want specific health related information from you or may have specific medication regimens or dietary suggestions for you. We can help to review your medications, when to use them, and what they are for. We can also help assemble the clinical information your doctors may want, such as vital signs, frequent weights, blood sugar readings, edema measurements, oxygen levels, and home safety records. We can help explain and support the Lifestyle Changes your Health Care Providers may be asking of you. Anything that can help your Health Care Providers take better care of you, and keep you Healthy and Safe at Home!

Ever have difficulty understanding your medications or what your doctor says? Do you have questions for your doctor you want answered? Do you need help getting to your medical appointments, or understanding what your Health Care Provider was saying? A member of our Health and Safety Team can help to explain your health issues, your medications, your health care provider’s instructions, accompany you to appointments, and advocate to your Health Care Provider questions or concerns you want to bring up.
